As some of you already know, I am writing to inform you that I will be retiring from general practice. My last consulting day will be Friday 22nd December 2023.
For patients requiring specialised prescribing, reception will supply a letter of alternative locations and support services you can contact to continue your care.
Your medical record will continue to be held securely at Jasper Medical. Either reception staff or I can arrange an introduction to another doctor for you to continue your care where applicable. I have attached a list of the doctors at the practice overleaf who are currently accepting patients.
I can also facilitate a summary of the important information in your file so that your new doctor has this material readily available at your first consultation. Please book an appointment with me to arrange this.
It has been rewarding to support you as patients, and I have enjoyed building relationships with you. I am looking forward to spending more time with my large family, travelling, and golfing.
