As many of you already know, I am writing to inform you that I will be retiring from general practice. My last consulting day will be Thursday 19th December 2019.
Your medical record will continue to be held securely at Jasper Medical. Either reception staff or I can arrange an introduction to another doctor for you to continue your care. I have attached a list of the doctors at the practice overleaf who are currently accepting patients.
I can also facilitate a summary of the important information in your file so that your new doctor has this material readily available at your first consultation. Please book an appointment with me to arrange this.

Making this decision has taken much consideration, and retiring from practice with so many wonderful patients will be difficult.
